*This is a Minimum Data Set (MDS) Coordinator position*

 

At Houston Methodist, the Minimum Data Set (MDS) Coordinator position is a licensed staff nurse, who as an experienced clinician, functions at the Competent to Proficient stage of Benner's model of clinical practice. This position is the subject matter expert and has complete knowledge of federal and state Resident Assessment Instrument (RAI) and Prospective Payment System (PPS) processes to meet regulatory and reimbursement compliance for the SNF (Skilled Nursing Facility) Unit. The MDS Coordinator position serves as a primary contact for the state and federal agency surveys, gathering and providing patient data, and completes federal forms. This position communicates and collaborates with the physicians and interprofessional health care team to educate, coordinate and maintain compassionate, efficient, quality care and achievement of desired treatment outcomes.

Responsibilities

PEOPLE ESSENTIAL FUNCTIONS

  • Provides education and mentoring to physicians and the inter-professional health care team regarding SNF State/Federal guidelines and regulatory compliance requirements including RAI documentation, PPS processes, requirements, changes and updates.
  • Develops and updates individualized comprehensive care plans for each patient as required. Uses therapeutic communication to establish a relationship with patients, families and inter-professional health care team to collaborate on the plan of care.
  • Provides staff education specific to patient populations and unit processes, coaches and mentors other clinical staff. Conducts audits to evaluate staff's compliance with required regulatory documentation.


SERVICE ESSENTIAL FUNCTIONS

  • Develops, updates and communicates plan of care, including discharge, in partnership with the patient, family and inter-professional health care team, using the nursing process. Prioritizes care based on the patient's needs, abilities and preferences.
  • Follows the patient and family-centered care standards, as outlined in the Houston Methodist Professional Nursing Practice document. Practices in a manner that is congruent with cultural diversity and inclusion principles.


QUALITY/SAFETY ESSENTIAL FUNCTIONS

  • Serves as a primary contact for the state and federal agency surveys. Gathers and provides patient data, completes federal forms and is the subject matter expert.
  • Participates in the Quality Assurance and Performance Improvement (QAPI) program by identifying areas for improvement and implements actions for resolution. Assists with collection of outcome data and reports through unit Case Management Performance Improvement (CMPI).
  • Abstracts patient data from the electronic heath record for completion and submission of the MDS. Reviews and analyzes information from external data sources (i.e., Certification And Survey Provider Enhanced Reports (CASPER), Program for Evaluating Payment Patterns Electronic Report (TMF PEPPER).


FINANCE ESSENTIAL FUNCTIONS

  • Ensures the overall completion and timely submission of the MDS documentation for each patient in the MDS system.
  • Analyzes and collects MDS documentation for accuracy and compliance with regulations. Identifies and resolves issues of non-compliance. Maintains all appropriate documentation.
  • Ensures Prospective Payment System (PPS) regulations are adhered to and communicates with appropriate departments for accurate coding and billing.
  • Oversees Payroll Based Journaling (PBJ) on a weekly basis (notifying and collaborating with other disciplines) to ensure accuracy of PBJ reporting. Prepares files to submit PBJ each month.


GROWTH/INNOVATION ESSENTIAL FUNCTIONS

  • Identifies and presents initiatives for innovation, efficiency and improvement in patient care and unit operations. Offers innovative solutions through evidence-based practice/performance improvement projects and shared governance activities.
  • Keeps abreast of all CMS regulatory changes related to IRFs and communicates changes/updates to management team.

Qualifications

EDUCATION

  • Bachelor's degree or higher in from an accredited school of nursing


WORK EXPERIENCE

  • Three years of registered nurse (RN) experience in acute or post-acute care
  • One year clinical experience with Resident Assessment Instrument/Minimum Data Set (RAI/MDS) experience

License/Certification

LICENSES AND CERTIFICATIONS - REQUIRED

  • RN - Registered Nurse - Texas State Licensure and/or Compact State Licensure within 60 days OR
  • RN-Temp - Registered Nurse - Temporary State Licensure within 60 days AND
  • BLS - Basic Life Support (AHA) AND
  • RAC-CT - Resident Assessment Coordinator-Certified Program (AAPACN) within 6 months

  • Possess current knowledge base regarding state and federal regulations relative to the RAI and MDS processes
  • Maintains knowledge of regulatory changes through external and internal training
  • Well versed in skills of the entire Microsoft Office Suite (Excel, Outlook, PowerPoint and Word)
